1. Types of Information We Collect
The information we collect on our Site and through the use of our Services generally falls into the following two categories: Personally Identifiable Information and Non-Personally Identifiable Information. If we do combine Non-Personally Identifiable Information with Personally Identifiable Information, the combined information will be treated as personal information for as long as it remains combined.
Personally Identifiable Information: Personally Identifiable Information refers to information that lets us know the specifics of who you are. The Personally Identifiable Information we collect may include: your name, date of birth, e-mail address, login name, passwords, etc. We request Personally Identifiable Information when you register on the Site (“Register”).

Mobile Devices: Your use of the Site through your mobile device may also include collection of information from your mobile device. For example, the Site may request your permission to collect location data and/or may request access to multimedia (photos or videos) stored on your mobile device. Location data is not required for use of the Site and you have the option of declining collection of geolocation data. If you do not wish for your location data to be shared with Supply Clinic, please respond accordingly when prompted on your mobile device, or visit the your mobile device settings.
2. Purpose of Collecting Your Information
Our primary goals in collecting information from and about you are to:
- provide you with the all features and functions of the Site and the Services;
- communicate with you regarding our Services;
- provide all visitors a smooth, efficient, and personalized experience while using our Site and/or Services.
We may also use Non-Personally Identifiable Information on an aggregated and anonymous basis to analyze Customer and/or Vendor behavior and to help us understand how visitors use our Site. We use this information to gather statistical information, improve the content of our Site, to make our Site easier to use, and we may use it to customize our advertising (but we will not provide Personally Identifiable Information to advertisers).

When you Register, we collect personal information that can identify you (“PII”) such as your full name, email address, mailing address, and telephone number. We also may collect PII from you when you contact us or participate in activities that may be available through our Services such as making purchases, creating a profile, or rating or reviewing products. The information that we collect varies depending upon how you use our Services. When you Register, you will need to provide us with your name, business name, position/title, phone number, email, password, billing address, shipping address, number of employees. Vendors may also be required to provide us with the number of years they have been in business, their Federal Tax ID, fax number, customer contact number, customer service hours, and free shipping threshold. Customers may also be required to provide us with the number of dentists, number of technicians, specialty/specialties, and dental license number (if applicable). Whether or not you choose to provide such information is completely your own choice. But if you choose not to provide the information we request, you may be unable to use certain services, offers, and content on our Site.
Please keep in mind that certain features on the Site give you an opportunity to interact with us and others. These may include reviewing and rating Products. When you use these features you should be aware that any information you submit, including your name, location and email address, may be publicly available to others. We are not responsible for any information you choose to submit through these interactive features.
Also, whenever you voluntarily disclose personal information on publicly-viewable web pages, that information can be collected and used by others. For example, if you post your email address, you may receive unsolicited messages. 4. How We Use Your Information
Our primary use of your information is to provide you with Services through the Site. Generally speaking, we do not shared your Personally Identifiable Information, or the Personally Identifiable Information of your child, with third-parties.

6. How We Safeguard Your Information
We use reasonable technological and physical safeguards to protect Personally Identifiable Information you provide to us from loss, misuse and unauthorized access, disclosure, alteration and destruction. We follow generally accepted industry standards to protect the information submitted to us, both during transmission and once we receive it. The Sites are not intended for children under the age of 13 and you must be over 18 to create an account with Supply Clinic. We do not knowingly collect personally identifiable information, nor do we share such information with third-parties, via the Sites from visitors under the age of 13.

Your information may be stored and processed in the United States or any other country where Supply Clinic and/or the Site has facilities, and by subscribing to our Services and/or using our Site you consent to the transfer of information outside of your country.

If you reside in California and you have provided us with personal information, you may request information about our disclosures of certain categories of personal information (if any) to third-parties for their direct marketing purposes. Such requests must be submitted to us using the email address below. Within thirty days of receiving such a request, we will provide a list of the categories of personal information disclosed to third-parties (if any) for third-party direct marketing purposes during the immediately preceding calendar year, along with the names and addresses of these third-parties. This request may be made no more than once per calendar year. We reserve our right not to respond to requests submitted other than to the address specified in this paragraph.

To improve the services it can offer you, Supply Clinic may opt to expand its capabilities for obtaining information about users in the future. Supply Clinic will update this privacy statement continually to ensure that you are aware of developments in this area. We will post those changes here so that you will always know what information we collect online on through our Site, how we use it, and what choices you have.
