This Privacy Policy discloses the practices of Supply Clinic, Inc. (“Supply Clinic”) concerning information we obtain by and through your use of the Supply Clinic website (the “Site”) and the services provided through the Site (the “Services”). The Services include providing a platform for dental professionals (“Customers”) to comparison shop dental and healthcare supplies (the “Products”) offered by various third-party suppliers (“Vendors”). Supply Clinic is committed to respecting your privacy and recognizing your need for appropriate protection and management of Personally Identifiable Information you share with us. The purpose of this privacy policy is to explain the types of information Supply Clinic obtains about visitors to our Site and users of our Site and Services, how the information is obtained, how it is used, and the choices you have regarding our use of, and your ability to review and correct, the information. By using the Site and/or the Service, you are accepting and consenting to the use of your information as described in this policy.

1. Types of Information We Collect

The information we collect on our Site and through the use of our Services generally falls into the following two categories: Personally Identifiable Information and Non-Personally Identifiable Information. Location data is not required for use of the Site and you have the option of declining collection of geolocation data. When you Register, you will need to provide us with your name, business name, position/title, phone number, email, password, billing address, shipping address, number of employees. Vendors may also be required to provide us with the number of years they have been in business, their Federal Tax ID, fax number, customer contact number, customer service hours, and free shipping threshold. Customers may also be required to provide us with the number of dentists, number of technicians, specialty/specialties, and dental license number (if applicable). Whether or not you choose to provide such information is completely your own choice. But if you choose not to provide the information we request, you may be unable to use certain services, offers, and content on our Site.

Please keep in mind that certain features on the Site give you an opportunity to interact with us and others. These may include reviewing and rating Products. When you use these features you should be aware that any information you submit, including your name, location and email address, may be publicly available to others. We are not responsible for any information you choose to submit through these interactive features.

Also, whenever you voluntarily disclose personal information on publicly-viewable web pages, that information can be collected and used by others. For example, if you post your email address, you may receive unsolicited messages.
